Does this apply to both monsters and players? What kind of passive defense? (Shield, hidden skill, etc?)
What kinda passive? Not sure what you mean, but anytime it PINGS* (Melee Passive) activates, the damage dealt on that hit will be 50% dmg of what it would been for both monsters and players (That's a yes to if it applies to both).

This applies to mana shield as well if you activate melee ping, it will half you mana loss.

This only applies to MELEE Passive. I dunno, you can go kick some zombies to find out.

Side notes, monster with shields in defense will also greatly reduce range attacks dmg dealt to them as well.

15~23 max damage + heavy armor effects versus 17 armor pierce
